Assessment and Reviewing Officer - fixed term for 12 months

Persona have an exciting opportunity for an Assessment and Reviewing Officer working closely with Bury Council to review the care and support packages within Persona’s Supported Living service.  We want people with the right attitude and who share our values who will fit in to our team, purpose and enhance it even more.

The role will require you to work flexibly with teams. Developing appropriate relationships with customers, their families and wider professionals. Reviewing customer needs to ensure that the service delivered meets their needs and working to ensure the service to customers is consistent, person centered and appropriate to the individual.

The role will include organising and chairing customer reviews and assessments, devising support plans, monitoring reviews for customers in line requirements and involving all key stakeholders. The successful candidate will be able to provide advice and information about preventative community-based resources, including voluntary, private and independent sectors, to customers and/or carers and signpost to these services wherever possible to meet need and identified outcomes and to assess the benefit of assistive technology to the individual and make recommendations.

This role will be home based with the requirement to travel to Persona services within Bury to carry out assessments and reviews.

The successful candidate will be confident using IT systems, have experience of working in care and support services, an eye for detail along with a flexible approach to the role.
